New Patient Registration

registration

The doctors welcome new patients who live within our practice area. Patients living within our practice boundary may register with the practice when the practice list is open.

The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

To register with the practice, please present your medical card (if you have one) or complete a patient registration form which can be obtained at reception. Please put as much information on it as possible, this speeds up the registration process. Your notes from your previous practice will then be requested to be sent here. This can take six weeks or longer.

New patients registering with the practice will be offered a health check within the first three months of registering.

Please contact reception for further information.
